Health Insurance:

As a concerned provider, we understand the importance of your health care and the cost.  Please take a moment to review the following information regarding your medical services. 

Considerable care has been taken in setting our fees.  Every effort has been made to ensure that our charges accurately reflect the complexity of care rendered and the skill and expertise required for your care.

As a courtesy to you, we file many forms of insurance including but not limited to: Blue Cross/Blue Shield, United Healthcare, Aetna, Tricare, Medicare, Viva, and Cigna.  It is your responsibility to provide accurate insurance information to ensure prompt payment.  Ultimately, all patients are responsible for their medical charges.  If no payment is received from the insurance company within 60 days, payment will be due from the patient.

It is your responsibility to verify with your insurance carrier prior to your appointment that your plan covers the services and physician you have chosen.  If your insurance requires a referral from your primary care physician, you must obtain this prior to you appointment. 

Medicaid:

Our office does accept Medicaid for pregnancy and gynecological visits by referral.  We only see GYN Medicaid patients for problems whose appointments have been scheduled by their primary care physician.  Please note: We do not accept Medicaid for yearly exams, and we do not accept Medicaid as secondary to any insurance except Medicare.  All pregnant Medicaid patients must enroll in our Gift of Life Program.

Gift of Life:

Every patient who is pregnant and on Medicaid, must see a care coordinator to enroll in the Gift of Life Program.  You must contact your care coordinator in order to schedule you initial visit to see the doctor or midwife.
