Co-pays:

All co-pays are required at the time of service.  There is an additional $20 charge for all co-pays not paid at the time of appointment.

Self Pay:

Patients that do not have insurance are considered self pay.  Payment in full is expected at the time of service.

Outstanding Balances:

Patients are expected to make payments on outstanding balances prior to appointments.  Please speak with one of our financial counselors at (334) 279-9333 extension #289 or #315 to make arrangements or you can do so at Pay My Bill Online.

Service Charges:

There is a $30 service charge for all returned checks.
