As a concerned provider, we understand the importance of your health care and the cost. Please take a moment to review the following information regarding your medical services.
Considerable care has been taken in setting our fees. Every effort has been made to ensure that our charges accurately reflect the complexity of care rendered and the skill and expertise required for your care.
As a courtesy to you, we file many forms of insurance including but not limited to Blue Cross/Blue Shield, United Healthcare, Aetna, Tricare, Medicare, Viva, and Cigna. We currently do not take Medicaid Family Planning or Medicaid for yearly appointments. We do, however, take Medicaid for pregnancy. It is your responsibility to provide accurate insurance information to ensure prompt payment. Ultimately, all patients are responsible for their medical charges. If no payment is received from the insurance company within 60 days, payment will be due from the patient.
It is your responsibility to verify with your insurance carrier prior to your appointment that your plan covers the services and provider you have chosen. It is your responsibility to bring your insurance card to your appointment. If your insurance requires a referral from your primary care physician, you must obtain this prior to your appointment.
