As a courtesy to you, we file many forms of insurance including but not limited to: Blue Cross/Blue Shield, United Healthcare, Aetna, Tricare, Medicare, Viva, and Cigna. It is your responsibility to provide accurate insurance information to ensure prompt payment. Ultimately, all patients are responsible for their medical charges. If no payment is received from the insurance company within 60 days, payment will be due from the patient.
It is your responsibility to verify with your insurance carrier prior to your appointment that your plan covers the services and provider you have chosen. It is your responsibility to bring your insurance card to your appointment. If your insurance requires a referral from your primary care physician, you must obtain this prior to you appointment.
